How Gaming Form check-ins work
Gaming Form is a short daily check-in that lets a player record how they feel today. It is not a test or a skill rating.
When the form appears
The form opens automatically when the player enters the website or app if:
- the team owner enabled Gaming Form in team settings;
- the player is in the starting lineup;
- the player agreed to participate;
- it is already 09:00 in the team's timezone;
- the player has not submitted or skipped today's form.
If the form is closed without an answer, it may appear again on the next entry during the same day.
Consent
On the first prompt, choose:
- Participate to allow daily self-ratings to be stored;
- Do not participate to opt out of Gaming Form.
Participation is voluntary. The answers do not automatically affect player ratings, search results, or roster decisions.
What the scores mean
Choose a value from 1 to 5 for every metric.

Values between 1 and 5 describe a state between the two endpoints. There are no correct or incorrect answers.
Submit or skip
- Submit saves all five selected scores for the current day.
- Skip today records the day as skipped and hides the form until the next team day.
Only one submission or skip can be recorded per day. A new day is determined by the team's timezone.
Who can see the data
The player, team owner, and team captain can view the self-ratings. Data is retained for up to 90 days.
